Joining our practice
Please complete the form below if you would like to join our practice.
It is important that you indicate whether you wish to join as a private patient or as an NHS patient.
If you wish to join our practice as a private patient, you will receive a phone call from one of our
reception team to arrange an appointment at your convenience.
If you wish to register as an NHS patient and you have an eligible postcode (DD9, DD10 or AB30) we can add your name to our waiting list. You may wait several months or longer to be seen and at times we may stop adding names to the waiting list, depending on capacity. Patients on the waiting list will be contacted when we are able to offer an appointment. If you apply to register as an NHS patient and the list is closed, we will let you know. We will not add patients with ineligible postcodes to our NHS waiting list.
All new patients will be asked for a refundable deposit of £20 at the time an initial examination appointment is arranged.
The deposit is payable for both adults and children but is capped at £50 for families. The deposit is retained by the practice if patients fail to attend their initial appointment.
